10/29/2023 EnchantaVaughn Approved Behavior: QMHPA arrived at the home to conduct individual session with the Chevelle. QMHPA inquired about her evening. Chevelle discussed with QMHPA her evening was going well. QMHPA informed Chevelle the topic of todayâs session which was to continue to assist her with being able to identify coping skills and understanding her mental health triggers. Intervention: QMHPA begin session by inquiring about Chevelle's emotional well-being. QMHPA educated Chevelle on how she needed to speak to her mother about the situation as oppose to getting angry over tings that she does not have any control over. QMHPA educated Chevelle on an old saying, âHe who controls you angers youâ. QMHPA provided Chevelle in a skill building activity on anger. QMHPA processed with Chevelle about how anger can cause us to lose our behavior and how we can calm our anger and stay in control. QMHPA engaged Chevelle in exercises demonstrating how to relax and calm down (breathing deeply, muscle relaxing, and how to combine the two). Chevelle was instructed to identify ways in which she handles her anger in positive ways. Chevelle was engaged in a role-play where anger was created. After processing the role-play Chevelle was instructed to discuss how controlling her anger could have changed the situation. QMHPA educated Chevelle on how she needs to continue to adhering to her medication to also control her mood stability. Response: Chevelle was open to suggestions and working on her anger for today's session. While exploring Chevelleâs current mood, she reported to QMHPA, she was feeling good. She verbalized to the QMHPA that she just like for someone to tell her do something and they do not do it themselves. Chevelle was able to identify ways in which she positively handles her anger. She explained ways in which she has not handled her anger appropriately such as incidents that happened between her mother and her. However, she was able to provide ways in which she could have handled his anger more appropriately in past situations. She appeared to be pleased with the role-play and was able to process the role-play appropriately without becoming angry during the session. Plan: QMHPA will continue to engage Chevelle in more role-playing activities assist her with learning how to respond composedly in stressful situations that may occur (1-2 times weekly).
